In the professionally structured everyday lives of many people in Berlin-Mitte, a familiar pattern emerges: diet is optimised, exercise is consistently integrated, routines are adjusted – yet the number on the scales barely changes.

In most cases, the problem is not in the implementation, but in the measuring instrument.

The scales record only total body weight. However, they do not differentiate between muscle mass, fat tissue, water or bone mass. This information is insufficient for a well-founded assessment of metabolic developments.

Weight and metabolic health are not identical

In nutrition counselling, body composition is an important additional parameter because total body weight alone does not allow differentiation between different body compartments.

A stable weight can simultaneously mean:

• reduction in fat mass
• increase in muscle mass
• adjustments in fluid balance

Especially with a structured change in diet or strength training, such changes are physiologically meaningful – but they remain invisible on conventional scales.

Physiological fluctuations are normal

Short-term weight fluctuations often arise from changes in water balance. The storage of carbohydrates as glycogen is associated with measurable water binding (Olsson & Saltin, 1970).

Hormonal changes, stress or training load can therefore influence body weight without an increase in fat occurring.

An isolated view of weight cannot differentiate these mechanisms.

The role of visceral fat

For a prevention-oriented strategy, it is not only the amount of fat that is important, but in particular its distribution.

Visceral fat is closely associated with metabolic and cardiometabolic risk profiles (Després, 2012; Fox et al., 2007; Lee et al., 2020).

The body mass index does not take this differentiation into account. A person may have a BMI within the normal range and still have an increased body fat percentage or an unfavourable fat distribution.

Structured body analysis at the Center for Prevention and Nutrition in Berlin-Moabit

At the Center for Prevention and Nutrition in Berlin-Moabit, a differentiated analysis of body composition based on bioelectrical impedance analysis (BIA) is carried out in addition to the medical history.

The BIA method is scientifically established and clinically validated (Kyle et al., 2004; Sergi et al., 2015).

However, what is crucial is not only the one-off measurement, but structured follow-up observation.

In our center, the following parameters are systematically recorded and documented:

• muscle mass
• body fat mass and body fat percentage
• visceral fat indicators
• total body water
• estimated basal metabolic rate

Through regular progress monitoring, changes can be tracked objectively. Progress becomes visible – independently of short-term weight fluctuations.

This form of differentiated observation enables precise adjustment of the individual strategy within nutrition counselling and prevention programmes.

Data-based support instead of isolated numbers

Meta-analytical data show that systematic monitoring significantly supports goal achievement (Harkin et al., 2016).

At the Center for Prevention and Nutrition in Berlin-Moabit, support is based on objective parameters. Decisions are made on a data-based basis – not solely on the basis of daily weight or subjective perception.

In addition to body analysis, the composition of the gut microbiome may also be considered as part of a holistic prevention approach – for example, through the Darm-Mikrobiom Plus Stuhltest, which provides information on the gut flora; the results do not replace a medical diagnosis, and medical advice should be sought in the event of abnormal findings.

Especially in the demanding professional environment of Berlin-Mitte, this structure creates clarity, transparency and confidence.

Summary

The scales alone are not a suitable instrument for assessing metabolic development.

For prevention-oriented support, analysis of body composition is considerably more informative.

At the Center for Prevention and Nutrition in Berlin-Moabit, structured body analysis enables an objective assessment of the current situation as well as traceable documentation of changes over time.

It is not total body weight alone that is crucial – but the composition of the body and the development of measurable parameters.
